You can download and fill out our New Client Forms New Client Form 1 & New Client Form 2 along with the form for which specialist you are seeing (see below), and bring it in at your first appointment to greatly expedite your check in process. We look forward to meeting you.

We operate under an appointment only basis.  Emergency cases are accepted 24 hours a day and shall  always receive top priority.
